A new Integrated Community Pulmonary Rehabilitation and Home Oxygen Assessment Service in Hounslow has created an exciting opportunity for a Healthcare Assistant (NHS Band 2 equivalent).
Working for an international provider of Respiratory Healthcare Services, you will immediately become a member of our successful, growing Homecare team and part of a network of specialists delivering our services across the UK.
Reporting to the Service Lead and working from a variety of community locations in Hounslow, your key responsibilities include:
Support the team in assessing and reviewing patients that have been referred to the service;
Conduct patient pre-screening by reviewing referral forms from referring clinicians, booking patient appointments where the referral meets agreed criteria or referring to the team where there is a query
Support the team as required during the course of a patient assessment
Maintain a secure Patient Register, organising assessment and review appointments
Maintain appropriate and current patient records
Liaise with patient families and their carers regarding the appointment, what to expect, travel to and from the service locations
Manage patients communication material
To support the Service Lead in:
Developing the service
Communicating and promoting the service locally
Ensuring the service is highly integrated within the care pathway
Propose and implement continuous improvements to the service
Run monthly KPIs reports for the purpose of measuring Service performance
Support our Clinical Governance programme within the service by:
Seeking, proposing and implementing continuous improvements to the service
Safeguarding high standards of care by following key components of our robust clinical governance system, including:
Clinical effectiveness

Research & Development
Education and Training
Evidence based healthcare
User/Patient involvement
Performance Appraisal and Continuous Professional Development
Understanding of the needs of a patient with a long term condition
Confident in using computer and electronic patient management software
Very good organisational skills
Commitment to patient-centred care
Excellent interpersonal and communication skills (verbal and non-verbal) with people from a wide variety of backgrounds/cultures
Evidence of recent Personal Development
To be highly motivated and highly motivating
Desire to constantly improve self and the service
Professional manner and approachable
Awareness of own limitations and scope of practice and when to seek help
Awareness of policies and procedures which effect clinical practice
Qualifications:
NVQ Healthcare Level 2/3 or equivalent
Experience:
Evidence of formal education with achievement of core literacy and numeracy skills
Demonstrate commitment to life long learning
Previous experience within a health care assistant role or equivalent within a healthcare setting
Excellent IT skills
Excellent communication and interpersonal skills
Innovative with an openness to constantly improve the programme
High patient orientation
Experience of patient administration systems

A full UK driving licence and a car are required for this role.
