The successful candidate will work as part of the busy Clayton District nursing Team, based in the Hyndburn locality.
Although you may be asked to work in any locality within the trust.
You will be required to work flexibly to cover the needs of the service and as part of a busy nursing team providing nursing care to patients in their own homes and care homes
We provide care seven days a week from 07:30hrs to 22: 00hrs on a rota basis
Duties will include nursing patients with complex and life limiting medical conditions in their own homes and care homes ranging from wound care, care of central venous catheters, end of life care.
Liaising and working alongside a range of other professionals/ agencies to promote holistic care to meet the needs of patients. Supported by senior staff members of the Integrated Team.
The successful candidate needs to be able to work as part of a busy team and also work unsupervised. Community experience, mentorship qualification and palliative care knowledge would be an advantage as would management of long term conditions.
Professional development opportunities include Non-Medical Prescribing, Mentorship and Specialist Practitioner qualification and also those with special interests can be Link Nurses in many areas including Infection Control, Tissue Viability, Continence, Diabetes, Palliative Care and Safeguarding.

As this position is exempted from the Rehabilitation of Offenders Act 1974 and involves work that is identified as a regulated activity as defined in the Safeguarding Vulnerable Groups Act (2006), any offer of employment is provisional upon a satisfactory DBS check being obtained.
The cost of an Enhanced Disclosure will be £48.00, which includes a small handling fee.
In line with other NHS organisations in the North West Region, the Trust is now passing the charge for undertaking a DBS check on to candidates in the event they are successfully appointed into the post for which they have applied. Candidates can choose whether to pay this over 1-3 months as a deduction from the monthly salary.
