To exercise a pivotal role in the liaison between primary, secondary and tertiary care and
between colleagues within the team; ensuring that the service is underpinned by timely and
accurate communication of information and is focused on the needs of each individual
To provide expert clinical case management to a specified group of patients their families and
communities within a defined population.

If you havent worked for three years or more you should provide us with a reference from your most recent employers; where this isnt possible we will accept references from someone of sufficient standing within your community who knows you, such as a doctor, solicitor, MP, religious leader/minister.
