Due to service redesign and developments an exciting opportunity has arisen for Registered (Adult) Nurses to join the Community Intermediate Care Team. This multi-disciplinary team provide a rapid response across the borough of Doncaster 24 hours a day 7 days a week to patients with health and/or social care needs and an out of hours community nursing service.

The aim of the service is to promote recovery from illnesses, avoid hospital admissions and support timely hospital discharges to maximise rehabilitation and independent living within a person’s usual place of residence, step or step down facilities.

We are seeking to appoint Registered Nurses who can demonstrate the vision and skills to work innovatively, flexibly and in partnership with other agencies supporting the MDT team in the assessment, implementation and evaluation of care in a variety of community/inpatient settings.

Essential requirements are a Registered Nurse (Adult) with appropriate demonstrable experience of working in both hospital and community settings. Flexibility and willingness to work a 24 hour rotational standard shift pattern including bank holidays and weekends.

Due to the need to travel between a multiple locations during a working day, sometimes carrying equipment it is essential that applicants are EITHER: A car driver OR able to be independently mobile in a suitable vehicle.

Desirable requirements are Mentorship/teaching training, Clinical Supervision training and
Intermediate Care and Rehabilitation experience.
