An exciting opportunity has arisen for a Registered Nurse to work with the current Parkinson’s Disease Nurse Specialist (PDNS), in Brighton and Hove.
The successful candidate should have an understanding of primary care or previous experience of working in the community, and be able to work autonomously as well as being part of a multi-disciplinary team. The position can be permanent part-time (as part of a job-share) or permanent full-time.
The post holder will be responsible for providing holistic assessment, planning, and implementing, and reviewing programmes of care for people with Parkinsonism’s, within their own homes and / or care homes. Hospital in-reach will also be part of the role. Working under the guidance of the PDNS, the post holder will work towards supporting patients in self-management, and independence, throughout the continuum of care. The post holder will help increase the capacity of the service, and pro-actively aim to reduce hospital admissions, by collaborative MDT working.
You need to have a UK driving licence and have access to a car for work purposes.
