An opportunity has arisen for a Staff Nurse Band 5 , to join a Nurse Led service providing person centred care to patients who are in various stages of organic type Dementia. You will be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care; and the maintaining of associated records. The post holder will work as part of a multi-disciplinary team to develop person centred holistic care, whilst maintaining individuals quality of life whilst assessing them for appropriate future placements. You will assist in the leading and monitoring of performance; supervising the work of other nursing staff, HCAs and student nurses. The post holder will take responsibility of the day to day management of the ward as required in support of the ward manager and deputy.
You will posses a Mental Health Nursing Diploma or Degree and have skills in delivering person centred dementia care and understanding of the CPA and Continuing Health Care processes/eligibility. You will be enthusiastic, motivated, have a desire to implement change in line with national and local modernisation of mental health services for Older People and possess excellent communication/ interpersonal skills.
