An exciting opportunity has risen to join our service in the community (Community Rapid Response Service). You will be an active member of the multi-disciplinary team composed of occupational therapists, physiotherapists, nurses, support workers and social workers, supporting patients at home to minimize risks and promote recovery through the promotion of independence and planned programmes of care with the aim of preventing unnecessary hospital admission.
As a Rapid Response Assistant, you’ll be responsible for delivering essential patient care under the supervision of a qualified practitioner, this includes:

- Providing personal care
- Relaying information
- Assisting with transfers and mobility
- Providing basic equipments
- Assessing and recording vital signs
- Assisting with feeding requirements
- Assisting with meal preparation, laundry and shopping etc.
- Treating all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences, both physical and cultural.
These are two permanent posts for x1.0 WTE x1 0.5 WTE with 6 months’ probation period covering Monday to Friday 08:00 - 20:00 and alternate weekends and bank holidays.
