Integrated Care 24 Ltd is a major not for profit Social Enterprise company providing primary care to over 4 million patients through a range of services which include GP Out of Hours Primary Medical Services, NHS 111, Primary and Secondary Care Support Services and Offender Health Services.
IC24 Overnight Community Nursing meets the nursing and night sitting needs within the Brighton and Hove CCG location between the hours of 8pm and 8am. Due to re -structuring we are looking for more team members.
The service provides both planned visits, referred through during the day, and unplanned visits during the night. Our main aims are to prevent unnecessary hospital admission, facilitate more expedient discharges from hospital or community beds and to support day time services to meet needs. We achieve this by running teams of qualified nurses and HCA’s based from Woodingdean. This is to provide either nursing visits to patient’s homes or community establishments or to input a 1-1 overnight support HCA.
This is a clinical position for a qualified nurse providing care for patients within the Brighton and Hove locality. The post holder will be required to deliver nursing care to a high standard and this may include initial assessment, care planning and risk assessment if they have not been performed by other community staff.
The emphasis is on ensuring that decisions made are based on good clinical practice, and inline within the prescribed protocols and both local and corporate policies.
