Salary: £16,099 - £18,691 plus working time payment of £2093.58 per annum

The Intermediate Care Service is a Joint Service funded and managed by the City of Edinburgh Council Department of Health and Social Care and the Edinburgh Community Health Partnership. It has been created from the review of three existing services, Community Rehabilitation, Rapid Response and Crisis Care.
The integrated service will be operational from early 2012 and we are now seeking to appoint additional team members to be in place for the launch of the new service; this will be an exciting opportunity for staff to work with a range of clients providing care, support and rehabilitation.
The service supports people over 16 to prevent hospital admission, support hospital discharge and promote independence in a range of activities of daily living involving specialist Occupational Therapy and Physiotherapy input.
The post requires shift work including evenings and weekends and involves extensive travel around the city in order to support people in their own homes and communities. Applicants will require to have experience of working with adults and/or older people in a care, support or rehabiltiation setting.
